FILE A CLAIM

Pursuant to the Court's Order Preliminarily Approving Settlement and Providing for Notice As Amended, filed on May 19, 2023, Class Members who wish to participate in the Settlement shall complete and submit the Proof of Claim and Release Form ("Claim Form") in accordance with the instructions contained therein. To obtain a copy of the Claim Form, click here. The deadline to submit a timely Proof of Claim is September 6, 2023.

If you fail to execute and submit a timely, properly completed and addressed Claim Form, your claim may be rejected and you may be precluded from any recovery from the Net Settlement Fund.
